Special Authorizations

Mental Health Management Group provides clinical evaluations and documentation services when appropriate for employment, educational, or disability-related purposes. These services are available following an individualized psychiatric evaluation conducted by a licensed healthcare professional.

Documentation may be considered for:

  • Short-Term Disability evaluations
  • Family and Medical Leave Act (FMLA) documentation
  • Workplace or school accommodation support
  • Emotional Support Animal (ESA) evaluations

All determinations are based on clinical assessment and professional judgment. Documentation is not guaranteed and is provided only when supported by evaluation findings.

Occupational and Educational Documentation

Evaluations for occupational or educational accommodations involve structured clinical review, including assessment of symptoms, functional limitations, and relevant history. For children and adolescents, information gathering may include parent or guardian input, school performance history, and prior evaluations when applicable.

If documentation is clinically appropriate, written reports or required forms may be completed in accordance with applicable regulations.

Timing and Scheduling

Because documentation requires a completed evaluation and clinical review, advance scheduling is recommended.

For new patients, a lead time of approximately one month prior to a submission deadline may be necessary to allow for evaluation, documentation preparation, and form completion.

Established patients are encouraged to submit requests at least one to two weeks before a due date, when possible. Timelines may vary depending on complexity and provider availability.

Emotional Support Animal (ESA) Evaluations

An Emotional Support Animal (ESA) evaluation involves clinical assessment of whether an individual has a qualifying mental or emotional condition and whether an animal provides support related to that condition.

ESA letters are issued only after personal evaluation and when clinical criteria are met. The letter documents that the individual has been assessed by a licensed healthcare professional and that, in the clinician’s judgment, the animal provides support related to the individual’s mental health needs.

Approval by airlines, housing providers, or other institutions is determined by those entities and may be subject to federal, state, or organizational policies.

Legal and Regulatory Considerations

Psychiatric disability is defined under federal law, including the Americans with Disabilities Act (ADA), as a mental impairment that substantially limits one or more major life activities. Determination of disability status requires clinical evaluation and is not assumed.

In accordance with Florida law and professional standards, supporting documentation for Emotional Support Animals cannot be provided without personal knowledge of the patient’s condition and appropriate clinical evaluation.
